I authorize any of the persons and organizations listed on this application to give you any and all information concerning my previous employment, education and qualifications for employment. I also authorize the Melvin J. Berman Hebrew Academy to request and receive such information.
If employed, I will abide by the policies and procedures of the Melvin J. Berman Hebrew Academy. I understand that acceptance of an offer of employment does not create a contractual obligation upon the employer to continue to employ me in the future. Upon termination, I authorize the release of reference information by the Melvin J. Berman Hebrew Academy.
I understand that as a condition of my employment, I will be required to apply for and pass a criminal history records check in Maryland as mandated by Maryland law and that as part of this process I will be required to submit legible fingerprints at a designated law enforcement location.

email@example.com, and she will forward to your previous employer for final completion. The completion of this form is a Maryland law, effective July 1, 2019.
